fix: watch resyncs index on every tick; add kqs resync

watch previously re-indexed only when auto-commit produced a new
commit, and only the files/FTS tables - trace graph, deletions and
phantom nodes were never touched. handle_debounce_tick now runs a
unified resync (files + FTS prune + trace rebuild from working tree)
on every tick, full for the knowledge repo and files-only for linked
projects. New 'kqs resync' command and best-effort resync after
doc/typespec/screen/userflow creation.

fix: clippy needless_borrow in state_dir call sites

fix: cross-platform signal handling in watcher (Windows ctrl_c)

refactor: rename kq:// scheme, markers, env and remaining brand refs …
