feat: data-driven preset — all Cake layers, plugin isolation, fixtures

Refactor the const-heavy CakeAppPreset into a data-driven catalog:
  - CakeLayer + CakeRule enums, LayerCatalog, PresetContext and six rule groups
  - add Mailer, Middleware, Form, Type, Route, Widget, CacheEngine, LogEngine,
    MailerTransport, Event, Policy, Authenticator, Identifier, PasswordHasher,
    UrlChecker layers (pattern + naming + isolation bans + quality + safety)
  - plugin support: pluginNamespaces registers per-plugin owner layers before
    the canonical ones; plugins are isolated from the app namespace by default,
    with per-plugin app access via pluginAppAccess
  - ClassNameSuffixRule excludedLayers fix: nested layers (Component vs
    Controller, etc.) are never forced into a parent suffix, even under a broad
    host Controller pattern

Tests: move to tests/TestCase, add tests/Fixtures with nine app/plugin fixtures
scanned end-to-end through the analyser against exact violation manifests.
