fix: repair the owners project-wide Fix All can, not none of them

Preparation was all-or-nothing: the first owner the planner could not handle
made planOwners return undefined, and the whole transaction came back
unfixable. A project holding a single genuinely unfixable finding therefore
offered a Fix All button that repaired nothing.

The v22 fixture project shows it exactly. Its audit reports nine findings:
eight uses of a pipe that one import closes, and one component that needs a
second import named SharedModule, which cannot be added without an alias.
Preparation refused all nine.

An owner that will not plan is now skipped and counted in skippedIssues,
and the result message names what was left behind. The hazards that make any
edit untrustworthy still abort everything: a client without atomic workspace
edits, an incomplete audit, a snapshot that disagrees with itself, and
cancellation. Order matters in the loop — the epoch and cancellation check
runs before the decision to skip, so a stale preparation cannot be mistaken
for an owner that merely would not plan.

feat: migrate extension to the Language Server Protocol

Diagnostics, completions, definitions, quick fixes, project discovery and indexing now
run in a dedicated language server; `src/extension.ts` does nothing but start and stop
the client. `ARCHITECTURE.md` describes the layers and the boundary.

Formatting configuration comes with it, and without Prettier. The server needed the
package at runtime for a single call — `resolveConfig`, for the four values the import
planner hands to ts-morph — which meant copying 8.3 MB of parsers this code never opens
into `dist/`. Those values are now read from the config files themselves: `.prettierrc`,
`.prettierrc.json`, a `prettier` key in `package.json`, and `.editorconfig`, with the
same cascade and precedence. A config written as code is detected but not executed —
running it would mean executing project code inside the server — so such a project gets
Prettier's own defaults, which is what it already got for every option it leaves unset.
Prettier stays a devDependency for `format:test-projects`, and `inspect-vsix` now
asserts the archive carries no `dist/node_modules` at all.
