Track initial round of feedback for upgradability & future proofing

• Merge the 3 gas params in 1, as discussed for API simplicity.
• EIP7702 Proxy for backwards incompatible updates
• DS-Proxy-style for backwards compatible updates (via ERC-7579)
• Make authorize / revoke replayable across-chains
• Ensure that we can sweep fees from EntryPoint
• Ensure that we can handle refunds for user from EntryPoint
• Forge script for deployment
• Allow ERC-1271 isValidSignature to allow certain keys to validate, if msg.sender is appropriate. If the key is revoked, this entire mapping is discarded.
